I declare and avow that the art I am submitting to HBIDA for the Art Contest is the original work of the artist named on this form. By signing this form I grant and assign the Houston Branch of the International Dyslexia Association (HBIDA) the non-exclusive right and permission to use, re-use, publish, re-publish, and otherwise reproduce and display the digital image submitted individually or in conjunction with other original submissions and to include the submission title, artist’s first name and grade level as identification. I hereby release and discharge HBIDA from any and all claims and demands arising out of or in connection with the use of the digital image, including without limitations any and all claims for libel or invasion of privacy. I have read and fully understand the contents of the Art Contest Entry and Submission and Release form represents and assigns. I further release HBIDA from any responsibility for injury incurred during the research or production of the original art submission.
